Union Dues Authorization

I hereby authorize Pacific Northwest Ironworkers Federal Credit Union to deduct my union dues out of my credit union account monthly:
** It is expressly understood that my union dues deduction will be paid from my Paid Time Off account, which also funds Sick Leave. You are advised the deduction will be deducted from the Paid Time Off account and affect the Sick Leave funds available you.

The withdrawal will be deducted from my account once a month. With a change in my status (apprentice to journeyman) or dues increase, this authorization will remain in effect unless written notice is giving revoking the deduction. In the event of insufficient funds in my account, it shall be my responsibility to pay my dues directly to the Union. This authorization shall remain in full force and effect until the month following a written notice revoking the deduction.
